Nurse Practitioner / Physician Assistant – Geriatric Care

Location: Seattle, Washington
Position: 1 Full-Time Position
Setting: Skilled Nursing Facilities (SNF)

Responsibilities

• Perform comprehensive histories, physical examinations, and ongoing assessments.
• Diagnose and manage acute and chronic medical conditions.
• Order, interpret, and document diagnostic studies and laboratory tests.
• Develop and implement evidence-based treatment plans.
• Prescribe medications and appropriate therapies.
• Coordinate specialist referrals and follow-up care.
• Educate residents and families regarding treatment plans, medications, and health maintenance.
• Collaborate with SNF nursing staff, therapists, social workers, peer providers, and supervising physicians.
• Provide consistent onsite daily patient care and respond to changes in resident conditions.
• Maintain accurate and timely clinical documentation.

Qualifications

• Current Washington State NP or PA license.
• Board certification/eligibility appropriate to the provider's specialty.
• Experience with geriatrics, skilled nursing, long-term care, or post-acute care preferred.
• Strong clinical assessment, communication, and care coordination skills.
• Ability to work independently while collaborating effectively with physicians and multidisciplinary teams.
