We are seeking Compassionate Licensed Clinicians: Licensed Psychologists (Ph.D./Psy.D.), Licensed Mental Health Counselors (LMHC), Licensed Clinical Social Workers (LCSW), to join our dynamic behavioral health team that provides services to the at need population of Assisted Living, Retirement, and Skilled Nursing Homes. In this vital role, you will be the preferred provider to a facility population that struggles with mental health issues. Having a heart to help Seniors who are aging in place, you will provide psychotherapy to residents and work with staff and adjacent providers in achieving a continuum of care.
Responsibilities
- Conduct diagnostic evaluations and behavioral assessments to identify proper diagnosis, crafting individual treatment plans and developing tailored intervention strategies.
- Conduct psychotherapy sessions for residents and family/significant others.
- Monitor residents mental health status, adjust treatment plans, communicate with staff.
- Create Progress Notes, Initial Evaluations, and ensure HIPAA compliance, along with completion of detailed EMR, record-keeping of medical records, and treatment notes.
- Collaborate with families, caregivers, and interdisciplinary teams to coordinate care plans and provide guidance on behavior management techniques.
- Maintain compliance with all regulatory standards related to patient care, confidentiality, and documentation requirements.
Experience
- Licensure as a Ph.D. or Psy.D. Psychologist, LMHC, LCSW; active state licensure must be maintained throughout employment.
- Demonstrated expertise in behavioral therapy techniques.
- Experience in progress note and initial evaluation patient assessment, and medical documentation within a healthcare or educational environment.
- Familiarity with HIPAA regulations and best practices for maintaining confidentiality of medical records and patient information.
