Social Worker

Employment Type: Full-Time / Part-Time
Schedule: Monday–Friday, Day Shift (8am-2pm, up to 8hrs)

We are seeking a compassionate and dedicated Medical Social Worker to join our Adult Day Health Care (ADHC) CBAS team. The Social Worker provides psychosocial support, counseling, advocacy, care coordination, and assistance with community resources to older adults and adults with disabilities.

Responsibilities

• Complete psychosocial assessments and identify participant needs.
• Participate in the development and monitoring of Individual Plans of Care.
• Provide counseling, supportive services, and participant advocacy.
• Assist participants and families with social, emotional, financial, housing, transportation, and community resource needs.
• Coordinate referrals to healthcare providers and community agencies.
• Participate in interdisciplinary team meetings and care planning.
• Monitor changes in psychosocial status and communicate concerns to the care team.
• Maintain accurate, timely, and confidential documentation.
• Provide crisis intervention and appropriate referrals when needed.
• Participate in quality improvement activities and comply with ADHC/CBAS regulations and facility policies.

Qualifications

One of the following educational backgrounds is preferred:

• Master's degree in Social Work (MSW) from an accredited school of social work.
• Master's degree in Gerontology.
• Master's degree in Counseling
• Bachelor's degree in Social Work (BSW) with at least two (2) years of experience working in a social services, healthcare, or related health care setting.

Additional Qualifications

• Experience working with older adults and/or adults with disabilities preferred.
• Experience in ADHC/CBAS, healthcare, community-based services, or case management preferred.
• Knowledge of community resources and care coordination.
• Strong communication, counseling, assessment, documentation, and organizational skills.
• Ability to work effectively as part of an interdisciplinary team.
• Compassionate and professional approach to working with participants and families.
• Ability to maintain confidentiality and comply with applicable HIPAA, California, and facility requirements.
