Physical Therapist (PT) – Part-Time

Pay Rate: $75 per visit
Benefits: No benefits
Bonus: $200 one-time bonus after 90 days, contingent upon meeting the required performance evaluation standard.

Position Summary

Triple Ace Home Health Agency is seeking a compassionate and experienced Physical Therapist (PT) to provide high-quality physical therapy services to home health patients. The Physical Therapist will evaluate patients, develop individualized treatment plans, provide skilled therapy interventions, and monitor patient progress toward functional goals.

Responsibilities

• Perform comprehensive physical therapy evaluations in the patient's home.

• Develop individualized treatment plans based on the patient's condition and physician orders.

• Provide skilled therapeutic exercises, gait training, balance training, transfer training, and mobility interventions.

• Assess and document patient progress and response to treatment.

• Educate patients and caregivers on home exercise programs, safety, fall prevention, and appropriate use of assistive devices.

• Communicate effectively with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team.

• Complete all required clinical documentation accurately and timely.

• Maintain compliance with Medicare, Medicaid, agency, and applicable regulatory requirements.

• Participate in care coordination and case conferences as needed.

Qualifications

• Current, unrestricted Physical Therapist license in the state of practice.

• Graduate of an accredited Physical Therapy program.

• Current CPR certification.

• Home health experience preferred.

• Strong clinical assessment, communication, and documentation skills.

• Reliable transportation and ability to travel to patients' homes.

• Ability to work independently while maintaining excellent patient care standards.

Compensation & Bonus

• $75/hour

• Part-time W-2 position

• No medical, dental, vision, retirement, or other employee benefits

• $200 one-time bonus after 90 days of employment, subject to successful performance evaluation and compliance with agency standards.

• Payroll is processed biweekly.
