As a LPN (Licensed Practical Nurse) on our evening shift, you will play a vital role in providing high-quality patient care and support to our residents. About the Role:
As an LPN, you will work closely with our interdisciplinary team to deliver individualized care to our patients, focusing on their unique needs and preferences.
You will administer medications, perform vital signs, and provide basic wound care, among other responsibilities.
You will also document patient information, maintain accurate records, and communicate effectively with patients, families, and healthcare providers.
Responsibilities:
Provide direct patient care, including medication administration, vital signs, and basic wound care.
Document patient information and maintain accurate records.
Communicate effectively with patients, families, and healthcare providers.
Participate in interdisciplinary team meetings to discuss patient care and concerns.
Assist with activities of daily living (ADLs) and maintain a safe and clean environment for patients.
Requirements:
Licensure: Current LPN license in the State of Wisconsin.
Education: LPN diploma or degree from an accredited nursing program.
Experience: Previous LPN experience in a long-term care setting preferred, but not required.
