MDS Coordinator
Job Description
We are seeking an experienced and detail-oriented MDS Coordinator to join our skilled nursing and long-term care team. The MDS Coordinator is responsible for coordinating the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of MDS assessments, and supporting compliance with federal and Texas state regulations.
Key Responsibilities:
Coordinate and complete Minimum Data Set (MDS) assessments in accordance with federal and state guidelines
Ensure timely and accurate submission of MDS assessments to regulatory agencies
Develop, review, and update resident care plans based on assessment findings
Collaborate with interdisciplinary team members to ensure quality resident care
Monitor and maintain compliance with CMS regulations and Texas state requirements
Participate in care plan meetings with residents and families as needed
Conduct audits to ensure documentation accuracy and regulatory compliance
Provide education and guidance to staff regarding MDS processes and documentation
Qualifications:
Registered Nurse (RN) license in the state of Texas
Minimum of 2 years of experience in a skilled nursing or long-term care setting
Prior MDS Coordinator experience required
Strong knowledge of RAI process, PDPM, and CMS guidelines
Strong knowledge of Texas Long Term Care Medicaid preferred
Excellent organizational, analytical, and communication skills
Ability to work independently and as part of a multidisciplinary team
