About the Role
As a Social Services Assistant at Spring Creek Rehab, you will:
Provide emotional support and counseling to patients and their families, addressing their social and emotional needs
Conduct assessments to identify patients' social and emotional needs, and develop personalized plans to address these needs
Facilitate communication between patients, families, and healthcare providers to ensure coordinated care
Coordinate with community resources and local organizations to provide patients with access to necessary services and support
Maintain accurate records and reports of patient interactions and progress
Requirements
1 year of experience in social services in long term care
High school diploma or equivalent required; degree in social work, counseling, or a related field a plus
Excellent communication, interpersonal, and problem-solving skills
Ability to work effectively in a fast-paced environment and prioritize tasks
Strong empathy and emotional intelligence
Ability to maintain confidentiality and handle sensitive information
