As a vital member of our clinical leadership, the MDS Coordinator (RN / LPN) plays a critical role in the accurate, timely completion of resident assessments and individualized care plans. This position is essential to ensuring our residents receive high-quality care while maintaining full alignment with regulatory standards and reimbursement guidelines.
Key ResponsibilitiesAssessment Management: Monitor, guide, and supervise the input and completion of MDS assessments in accordance with state and federal regulations.
Compliance & Submission: Meticulously review assessment data, transmit MDS assessments to the state, obtain validation receipts, and resolve any submission issues.
Care Planning: Schedule and facilitate quarterly and annual care plan conferences involving residents, families, and the interdisciplinary team.
Interdisciplinary Collaboration: Work closely with facility departments to ensure all clinical documentation accurately supports the MDS process.
Training & Mentorship: Serve as the subject matter expert on MDS processes, offering guidance, education, and support to facility staff.
Record Keeping & Regulation: Maintain complete records of assessments and care plans while staying current on evolving MDS guidelines.
Additional Duties: Execute clinical support tasks as assigned by the Director of Nursing and Administrator.
Licensure: Active Illinois Registered Nurse (RN or LPN) license in good standing.
Experience: Prior MDS assessment experience in long-term care or skilled nursing is required.
Regulatory Knowledge: Solid understanding of Case-Mix, Federal Medicare PPS processes, and reimbursement frameworks.
Technical Proficiency: Strong computer skills with hands-on experience using Electronic Health Record (EHR) platforms.
Core Competencies: Detail-oriented, highly organized, and able to manage complex workloads independently.
Interpersonal Skills: Clear communication skills to lead care meetings and collaborate across multidisciplinary teams.
