Location: The Gardens at Camp Hill – Camp Hill, PA
Schedule: Full-Time

The Gardens at Camp Hill is seeking a dedicated Social Services Director to lead the Social Services Department within our nursing home. This role serves as a liaison and advocate for residents’ social interests and is responsible for planning, organizing, implementing, evaluating, and directing the overall operations of the Social Services Department.

Position Overview

The Social Services Director ensures that residents’ medically related social and emotional needs are identified and addressed through comprehensive assessments, individualized care planning, and ongoing support. This position plays a vital role in resident adjustment, family communication, crisis intervention, and regulatory compliance.

Key Responsibilities
  • Develop comprehensive social assessments and individualized care plans that identify medically related social and emotional needs, establish realistic goals, and outline specific interventions.

  • Actively participate in pre-admission and admission processes.

  • Serve as a compassionate resource to residents and family members during periods of adjustment, crisis, or special need.

  • Document residents’ behavioral and psychosocial status accurately and timely.

  • Provide individual and group support services to residents and families.

  • Establish, coordinate, and support a volunteer program designed to meet specific social service needs.

  • Orient residents and families to the nursing center and assist with transition and adjustment to the new environment.

  • Ensure compliance with federal, state, and local regulations governing social services in long-term care settings.

Qualifications
  • Bachelor’s Degree from an accredited school in Social Work or a related field required.

  • Strong knowledge of social service practices, procedures, laws, regulations, and guidelines governing long-term care.

  • Excellent communication, organizational, and interpersonal skills.

  • Ability to work independently while managing multiple priorities effectively.
