Job Summary:

At SYNERGY HomeCare Of The West Valley, we are seeking a compassionate and dedicated Home Health Caregiver to join our team. As a Home Health Caregiver, you will play a vital role in providing high-quality care to our clients in the comfort of their own homes in Phoenix, Arizona. Our goal is to enable our clients to maintain their independence and improve their overall well-being, and we're looking for a passionate and skilled professional to help us achieve this mission.

Responsibilities:

• Provide personal care assistance to clients, including bathing, dressing, grooming, and toileting

• Assist with daily living activities, such as meal preparation, light housekeeping, and laundry

• Assist clients with medication management, including administering medications as directed

• Monitor and report any changes in clients' condition to our clinical team

• Maintain a safe and clean environment for clients, including their homes and personal belongings

• Build strong relationships with clients and their families, providing emotional support and companionship

• Collaborate with our interdisciplinary team to develop and implement individualized care plans

Requirements:

• Home Health Caregiver certification or willingness to obtain within a specified timeframe

• High school diploma or equivalent required

• Strong communication and interpersonal skills

• Ability to lift, transfer, and move clients with care and safety

• Reliable transportation and a clean driving record

• Ability to work varied shifts, including evenings, weekends, and holidays
