Fieldstone Nursing and Rehabilitation Center is looking to hire a Licensed Nursing Home Administrator (LNHA) to join our team. Fieldstone is nestled away in a beautiful area of Fostoria, Ohio, and is a 64-bed skilled nursing and rehabilitation facility.

Duties and Responsibilities:

  • Responsible for the leadership, oversight, and daily operations of the facility.

  • Oversee day-to-day operations of all facility departments and services, and ensure that compliance is ongoing.

  • Development and implementation of a facility management system.

  • Monitoring and maintaining the facility's compliance with all pertinent state and federal regulations, including Ohio Department of Health and CMS requirements.

  • Monitor monthly performance of the facility against budget and intervene as needed.

  • Lead census development and maintain hospital, physician, and managed care referral relationships.

  • Recruit, develop, and retain department heads and facility staff.

  • Makes sure the facility is a safe, clean, comfortable, and appealing environment for residents, patients, families, and staff according to company guidelines.

  • Complete required forms and documents following company policy and state and federal regulations.

  • Manage all aspects of the state and federal government survey process, including plan of correction and QAPI.


Qualifications and Skills:

  • Active Licensed Nursing Home Administrator license in Ohio (BELTSS)

  • Skilled nursing or long-term care administrator experience preferred, and recent AIT graduates are encouraged to apply

  • Must be highly organized, professional, and motivated

  • Excellent written and verbal communication skills

  • Knowledge of Medicare and Medicaid procedures and applicable policies

  • Extensive knowledge of current federal, state, local, and corporate guidelines and regulations governing management in long-term healthcare facilities

  • Reports to the Governing Body
