As our Director of Social Services, you will oversee the Social Services Department and help ensure residents receive the emotional, social, and psychosocial support they need.
Responsibilities include:
Direct, establish, and oversee Social Services Department policies and procedures
Maintain accurate and timely documentation
Participate in care plan meetings and advocate for residents' needs
Educate residents and families regarding Advanced Directives
Coordinate and support smooth admissions and discharges
Collaborate with nursing, therapy, administration, and other members of the interdisciplinary team
Provide leadership and support to the Social Services team
Maintain compliance with applicable regulations and facility policies
NJ LSW or CSW required
LCSW also welcomed
Long-term care experience preferred, but not required
Previous staff management experience preferred
Strong communication, organization, and leadership skills
Ability to work independently and effectively within an interdisciplinary team
