We are currently seeking a detail-oriented, dedicated MDS Coordinator Nurse to join our interdisciplinary healthcare team and lead the assessment and care planning process.
Job Summary:
As the MDS Coordinator Nurse, you will oversee the complete Minimum Data Set (MDS) assessment process, care planning, and care coordination for our residents. Working closely with nurses, therapists, physicians, and families, you will ensure accurate clinical assessments, regulatory compliance, and resident-centered care delivery.
Key Responsibilities:
• Conduct initial, comprehensive, and ongoing MDS assessments to evaluate resident needs and establish individualized care plans.
• Coordinate with interdisciplinary healthcare teams to ensure seamless care transitions and full compliance with state and federal regulations.
• Develop, implement, and update comprehensive care plans in collaboration with nurses, therapists, and care providers.
• Lead and participate in resident care conferences and complex case reviews to track progress and adjust care plans accordingly.
• Maintain complete, accurate, and timely documentation of assessments, care plans, and care delivery.
• Collaborate with families, caregivers, and medical staff to promote a resident-centered care environment.
• Identify and report any changes in resident conditions or clinical concerns to the interdisciplinary team.
Ideal Candidate Qualifications:
• Licensure: Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Ohio (Required).
• Experience: Minimum of 1 year of MDS or long-term care / skilled nursing experience preferred.
• Skills:
+ Strong working knowledge of MDS processes, RAI guidelines, and care planning standards.
+ Excellent organizational, time-management, and clinical documentation abilities.
+ Strong communication and interpersonal skills to collaborate effectively across departments.
+ Proven ability to thrive in a fast-paced environment and handle sensitive resident information with discretion and HIPAA compliance.
