- Assess the medical and psychosocial needs of patients effectively and empathetically.
- Provide patients with information, referrals, and linkage to essential services.
- Conduct outreach and follow-ups to ensure comprehensive care.
- Advocate passionately for patients to bridge service gaps.
- Empower patients with education and tools for self-management.
- Monitor and evaluate the quality of services provided.
- Participate actively in Performance Improvement and Quality Assurance Programs.
- Attend specialized training for Medicaid's Case Management for Children and Pregnant Women Program.
- Maintain a direct patient care caseload within Behavioral Health.
- Conduct appointments and provide clinical supervision.
- Respond to Risk Assessment calls and attend community coalition meetings.
- Onboard and mentor new Social Service staff.
- Travel between locations as needed to provide support.
- Support walk-in patients and maintain work area and equipment.
- Perform other duties as assigned to support the team and mission.
- MSW from a CSWE accredited institution required.
- Active licensure as defined by the Texas State Board of Social Worker Examiners.
- BLS/CPR Certification.
- Experience working with children with special needs and pregnant women is desirable.
- Knowledge and adherence to the NASW Code of Ethics.
- Effective communication with diverse patients and stakeholders.
- Excellent interpersonal and communication skills.
- Proficient in Microsoft Office suite.
- Experience with community agencies and case management.
- Knowledge of public benefits and access facilitation.
- Able to manage multiple deadlines and decision-making responsibilities.
- Bilingual in English/Spanish preferred.
