Job description

We are seeking a dedicated and experienced Physician to provide comprehensive medical care in a Skilled Nursing Facility (SNF) in South Bend, IN. The Physician will play a critical role in assessing, diagnosing, and managing the healthcare needs of residents, ensuring high-quality care in a long-term care setting. This position requires collaboration with Nurse Practitioners, nursing staff, and other healthcare professionals to enhance patient outcomes and promote overall well-being.

Duties and Responsibilities

  • Perform comprehensive medical assessments and develop individualized treatment plans

  • Diagnose, manage, and treat acute and chronic medical conditions

Skills and Qualifications

  • Active Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) license in the state of Indiana

  • Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics

  • Experience in Skilled Nursing Facility (SNF), long-term care, or geriatric medicine preferred

  • Strong clinical assessment and diagnostic skills

  • Ability to work independently while collaborating with a multidisciplinary team

  • Excellent communication and interpersonal skills
