Job Description
UNM Medical Group, Inc. is hiring a Pharmacist Clinician to join our Primary Care Grande Clinic. This opportunity is a part-time (0.5FTE 20 hours a week) day shift opening located in Rio Rancho, New Mexico.
Minimum $114,168 - Midpoint $145,468*
*Salary is determined based on years of total relevant experience.
*Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.
Summary:
The Pharmacist Clinician provides direct patient care through comprehensive medication management
and optimization of pharmacotherapy. Working collaboratively with physicians, advanced practice
providers, nurses, and other healthcare professionals, the Pharmacist Clinician evaluates medication
regimens, manages chronic and acute disease states, monitors therapeutic outcomes, and promotes
safe, evidence-based medication use.
In addition to advanced clinical responsibilities, the Pharmacist Clinician performs the essential
functions of a Staff Pharmacist, including medication dispensing, prescription verification, patient
counseling, medication safety oversight, and compliance with applicable regulatory and accreditation
standards. The Pharmacist Clinician serves as both a clinical resource and a practicing pharmacist,
ensuring the safe and effective delivery of pharmaceutical care across the continuum of care.
The Pharmacist Clinician functions as an integral member of the interdisciplinary care team and
provides services under collaborative practice agreements, clinical protocols, and applicable state and
federal regulations. This role balances direct patient care, clinical consultation, medication
management, and operational pharmacy responsibilities to support organizational quality, safety, and
patient outcome goals.
Minimum Job Requirements of a Pharmacist Clinician:
Education:
• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited college or
school of pharmacy
• Completion of an accredited PGY-1 Pharmacy Residency program within six (6) months of
employment
Experience:
• Two (2) years of directly related experience
• Experience practicing under a Collaborative Practice Agreement (CPA) preferred.
Certifications/Licenses:
• Licensed Pharmacist in State of New Mexico or obtain within 90 days
• Licensed Pharmacist Clinician in State of New Mexico or obtain within 180 days
• Board Certification through the Board of Pharmacy Specialists (BPS) including
Pharmacotherapy Specialist (BCPS), Ambulatory Care Pharmacist (BCACP) or other specialty
certification preferred.
Duties and Responsibilities of a Pharmacist Clinician:
1. Provides direct patient care to include, writing, renewing, and discontinuing prescriptions;
documenting clinical interventions and patient encounters in the electronic health record;
maintaining accurate, legible, and confidential records.
2. Conduct comprehensive medication reviews and assessments; develops, implements, monitors,
and adjusts medication therapy plans based on patient-specific needs and evidence-based
guidelines; identifies, prevents, and solves medication-related problems.
3. Evaluates laboratory values, diagnostic tests, and clinical data to optimize medication therapy;
monitors therapeutic outcomes and adverse drug events; provides patient education and
counseling regarding medications, disease management, adherence, and self-care.
4. Supports and creates a culture of service excellence, teamwork, and accountability in alignment
with ASPIRE values; collaborates with physicians, advanced practice providers, nurses, and other
healthcare professionals to improve patient outcomes; participates in interdisciplinary patient care
conferences and care planning activities.
5. Supports quality improvement initiatives and organizational performance goals; assists in the
development and implementation of clinical pathways, treatment protocols, and medication
management programs; monitors and reports quality metrics related to medication management
and patient outcomes; participates in medication safety and utilization review activities.
6. Participates in the development of clinical protocols or order sets to ensure the appropriate and
safe delivery of medications to patients; adheres to the appropriate mechanisms of consulting with
the supervising physician for pharmacist clinician protocols.
7. Serves as a medication resource for providers, staff, patients, and caregivers; provides drug
information and evidence-based therapeutic recommendations; may be asked to assist with
primary care team clinical inbox management; participation in education and training of pharmacy
residents, students, and healthcare staff as appropriate; maintains professional competency
through continuing education and professional development.
8. Promotes the most cost-effective delivery of medication therapy and use of medications to
patients; develops and implements programs to promote cost-effective alternatives and track their
effectiveness; evaluate opportunities to decrease pharmacy and other health care professional’s
time to provide optimal pharmaceutical care; evaluates opportunities and implements programs to
decrease waste or improve reimbursement of medication therapies.
9. Adheres to all applicable federal, state, and organizational regulations, policies, and procedures;
maintains patient confidentiality and compliance with HIPAA requirements; support a culture of
patient safety by adhering to safety practices, identifying and reporting risks or concerns,
improving processes, and encouraging patient involvement in their care.
10. Performs other related duties and responsibilities as assigned.
