Position Summary

The Social Worker is responsible for addressing the psychosocial, emotional, and social needs of residents while serving as an advocate and resource for residents and their families.

Working closely with the interdisciplinary team, the Social Worker helps develop individualized care plans, provides emotional support, coordinates services, assists with discharge planning, and promotes resident rights, dignity, independence, and quality of life.

What You'll Do
  • Complete psychosocial assessments, care plans, progress notes, and other required documentation

  • Assess residents' psychosocial and emotional needs and develop appropriate interventions

  • Participate in interdisciplinary care plan meetings and contribute to individualized plans of care

  • Provide counseling, emotional support, and guidance to residents and families

  • Help residents adjust to long-term care, rehabilitation, changes in health status, and other life transitions

  • Advocate for resident rights, dignity, privacy, independence, and quality of life

  • Coordinate family meetings and encourage meaningful family involvement in care planning

  • Assist with discharge planning and help coordinate safe transitions of care

  • Connect residents and families with appropriate community resources and support services

  • Maintain communication with residents, families, legal representatives, physicians, and members of the healthcare team

  • Collaborate with nursing, therapy, activities, dietary, and other departments to address residents' needs

  • Assist residents who may have limited or no family support by identifying and coordinating appropriate resources

  • Educate residents and families regarding available services, resources, and resident rights

  • Maintain accurate, timely, and confidential documentation

  • Build positive relationships with residents, families, staff, healthcare providers, and community agencies

  • Ensure responsibilities are completed in accordance with applicable federal, state, and facility requirements

What We're Looking For
  • Bachelor's degree in Social Work or a related human services field; Social Work preferred

  • Previous healthcare or long-term care experience preferred

  • Strong communication, organization, and interpersonal skills

  • Excellent documentation and case management abilities

  • Ability to maintain confidentiality and exercise sound professional judgment

  • Compassionate and resident-centered approach

  • Strong advocacy skills and a genuine desire to help others

  • Ability to work independently while collaborating effectively with an interdisciplinary team

Preferred Qualifications
  • Experience in a skilled nursing or rehabilitation setting

  • Familiarity with MDS assessments and interdisciplinary care planning

  • Social Work licensure or eligibility for licensure, where applicable
