About the Role:

We are seeking a highly skilled and compassionate Registered Nurse MDS Coordinator to join our care team. As a Registered Nurse MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) process, ensuring accurate and timely completion of assessments, and developing and implementing individualized care plans. You will also be responsible for supporting our multidisciplinary team in providing comprehensive care to our residents.

Responsibilities:

  • Coordinate the MDS process, ensuring timely and accurate completion of assessments and care plans

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Conduct assessments, gather data, and document resident information accurately and thoroughly

  • Collaborate with healthcare professionals to identify and address resident needs

  • Develop and maintain relationships with residents, families, and staff to promote open communication and collaboration

  • Participate in quality improvement initiatives to enhance resident care and satisfaction

  • Maintain accurate and up-to-date records and reports

Requirements:

  • Current registered nurse (RN) license in the state of Illinois

  • Experience in long-term care or post-acute care setting preferred

  • Strong clinical and assessments skills, with ability to accurately and thoroughly document resident information

  • Excellent communication and interpersonal skills, with ability to work effectively with residents, families, and staff

  • Strong organizational and time management skills, with ability to prioritize tasks and meet deadlines

  • Ability to work in a fast-paced environment and adapt to changing priorities
