Overview:

Correctional Healthcare Partners (CHP) is seeking a Discharge Planner to coordinate discharge and continuity-of-care planning across CHP's East Mesa and Youth Transition Campus facilities.

Position Summary:

Under the direct supervision of the Health Services Administrator (HSA), the Discharge Planner coordinates release and transfer planning, ensuring continuity of care for patients transitioning out of CHP's care.

Key Responsibilities:

Coordinate discharge planning and continuity-of-care arrangements for patients upon release or transfer

Communicate and collaborate with facility staff, medical providers, and community partners to ensure smooth transitions

Respond proactively to shifting schedules, directives, and patient needs

Partner with clinical and case management teams on youth-specific discharge needs

Maintain accurate, timely documentation of discharge plans and follow-up

Wishlist (in order of priority):

Prior experience working with youth

Correctional healthcare background

Prior discharge planning experience is a plus

Proactive and able to follow through on directives that may fluctuate and change; flexible and adaptable

Strong teamwork orientation

Additional Details:

Location: East Mesa and Youth Transition Campus (Kearny Mesa) — 446 Alta Rd Suite 1600, San Diego, CA & 2801 Meadowlark Drive, San Diego, CA (approximately half time at each site)

Schedule: M–F, 8:00 AM–4:30 PM preferred. Alternative options include a 4x10 schedule (Mon–Thu, 8:00 AM–6:30 PM) or M–F, 9:00 AM–5:30 PM. For exceptional candidates, a 12-hour x 3-day schedule may be considered.

Classification: Full-time

Status: Full-time

Compensation: $27–$30/hour
