GEM Transitional Care Center is seeking a motivated and experienced rehabilitation professional to join our team as an Assistant Director of Rehabilitation (ADOR).

We are open to candidates from the following disciplines:

  • Physical Therapist (PT)

  • Occupational Therapist (OT/OTR)

  • Physical Therapist Assistant (PTA)

  • Certified Occupational Therapy Assistant (COTA)

Candidates must hold a current and active California license in their respective discipline.

About the Role

The Assistant Director of Rehabilitation works closely with the Director of Rehabilitation to support the daily operations of the rehab department while helping ensure high-quality patient care, strong clinical outcomes, regulatory compliance, and an effective rehabilitation team.

Key responsibilities include:

  • Department Operations: Assist with daily department operations, including scheduling, staffing, caseload management, and workflow.

  • Patient Care: Maintain an active clinical caseload and provide skilled therapy services within the scope of your professional license.

  • Leadership: Support, mentor, and provide guidance to rehabilitation staff while promoting teamwork, accountability, and effective communication.

  • Clinical Oversight: Assist with monitoring therapy documentation, treatment plans, progress, and discharge planning to support appropriate and individualized patient care.

  • Quality Improvement: Participate in quality improvement initiatives and help identify opportunities to improve clinical outcomes and department performance.

  • Interdisciplinary Collaboration: Work closely with nursing, physicians, social services, case management, and other members of the interdisciplinary team.

  • Compliance: Support compliance with Medicare requirements, state and federal regulations, facility policies, and rehabilitation documentation standards.

  • Director Support: Assist the Director of Rehabilitation with department responsibilities and provide leadership support when needed.

Qualifications
  • Current and active California PT, OT/OTR, PTA, or COTA license

  • Experience working in rehabilitation; skilled nursing or post-acute care experience preferred

  • Strong clinical, organizational, and communication skills

  • Ability to work collaboratively with an interdisciplinary team

  • Leadership or supervisory experience preferred

  • Knowledge of rehabilitation documentation and Medicare requirements preferred

  • Commitment to providing high-quality, patient-centered care
