We are seeking a Social Services Director to work at our Long-Term Care facility located near North East Cleveland, Ohio. Position Overview:
As an LSW, you will help oversee care planning and ensure residents’ psychosocial needs are met with compassion and professionalism. You’ll act as a vital bridge between residents, families, facility staff, and community resources.

Responsibilities:
- Assess and address the social and emotional needs of residents
- Develop and manage individualized care and discharge plans
- Serve as a liaison among residents, families, medical staff, and external agencies
- Maintain accurate documentation, including MDS 3.0 and CAA’s
- Advocate for residents' rights and support ethical care practices
- Coordinate psychological services and hospice referrals as needed
- Participate in interdisciplinary care planning and quality improvement efforts
- Ensure compliance with state and federal regulations regarding social services
What We’re Looking For:
- Licensed Social Worker (LSW) – Required
- Experience in a skilled nursing or long-term care setting strongly preferred
- Strong communication, organizational, and leadership skills
- Knowledge of community resources and elder care best practices
- A passion for working with seniors and making a meaningful impact
