Job Responsibilities:
Provide direct patient care to patients with various medical conditions, including rehabilitation, postsurgical, and chronic care needs
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Administer medications, treatments, and therapies as prescribed by physicians and other licensed healthcare professionals
Monitor patient progress and adjust care plans as needed
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
