Hybrid APRN

Home Physicians Group is seeking an experienced Family Practice or Internal Medicine ARNP/APRN or Physician Assistant to join our hybrid remote and in-home care team.

The ideal candidate will provide high-quality, patient-centered care through virtual visits for homebound patients, along with in-home visits 1-2 times per week.

Founded in 2005, Home Physicians Group is a well-established Central Florida practice with more than 30 providers dedicated to caring for the geriatric population.

We provide mobile medical care across 14 Central Florida counties, serving patients in:

• Skilled nursing facilities
• Assisted living facilities
• Independent living communities
• Patients’ homes

Our providers work closely with patients, families, facilities, and care teams to improve outcomes, promote continuity of care, and reduce hospital readmissions.

Requirements :

• Duly licensed provider in the State of Florida, with unrestricted licenses to practice
• Current DEA license for APRN
• Proven experience as a APRN
• Professional bedside manner
• In-depth knowledge of legal medical guidelines and medicine best practices
• Excellent oral and written communication skills
• Bilingual preferred
• 1+ years as APRN or PA experience
• Responsible for the assessment, diagnosis, and treatment of assigned patients
• Administer nursing care to ill, injured, or disabled patients
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members
• Familiarity with medical software and equipment
• Ability to build rapport with patients and facilities
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
