Registered Nurse, Case Management – Grand Junction, CO, United States. This travel assignment starts on 08/31/2026 for an 8-week contract. Guaranteed hours are 36 per week, with a weekly pay range of $1,883 to $2,030. Shift: Full-Time. Work occurs in a healthcare setting, focusing on the development, implementation, and evaluation of the CWHS Case Management Plan to meet patients’ health and human service needs.

Responsibilities
- Under general supervision, develop, implement, and evaluate the CWHS Case Management Plan.
- Assess, plan, implement, coordinate, monitor, and evaluate the options and services required to meet the patient’s health and human service needs.
- Provide advocacy, communication, collaboration, resource management, and education to interdisciplinary team members.
- Allocate resources effectively to support high-quality, cost-conscious patient care; ensure utilization aligns with regulations and policies.
- Document Case Management plans clearly; attend and participate in departmental and interdepartmental meetings related to Case Management/Social Services.
- Collaborate with payers and review organizations to ensure proper utilization of resources; educate physicians and staff on appropriate level of care and utilization issues.
- Maintain current knowledge of Case Management, discharge planning, and utilization processes through continuing education.
Requirements
- Current unrestricted Registered Nurse license in Colorado
- Bachelor’s Degree in Nursing (Master’s degree preferred)
- At least three (3) years of clinical experience in a hospital setting
- At least five (5) years of case management experience in a hospital setting
- Certification in case management preferred or willingness to obtain within two (2) years of employment
- Ability to document Case Management plans clearly and to communicate effectively with physicians, patients, families, staff, and payers
- Knowledge of discharge planning, utilization processes, and continuum of care
- Ability to work in a fast-paced, collaborative care environment
