Position Description:


We are looking for a very organized and task-oriented Navigator who is well equipped to work with

families with children as well as vulnerable single women and men on a flexible schedule.



Responsibilities include but are not limited to:


1.Carry out the mission, vision and values of City Care.

2.Know the resources available in the community and develop a network of contacts.

3.Coordinate school enrollment for children accompanying families.

4.Develop individualized plans with families by working to identify their self-directed goals. Improve

resident outcomes by collaborating to develop, monitor, and evaluate service plans.

5.Provide case management assistance, resource development, crisis intervention and direct support

to families both in the City Care shelter and in various places in the OKC community. This includes

but is not limited to document recovery, collaboration with OKC’s Continuum of Care and other

partner agencies to leverage resources to facilitate progress and forward motion.

6.Promote housing stability for each incoming referral by documenting progress, resource linkages,

and contacts in the electronic record.

7.Ensure ROI’s, case notes and weekly documentation is updated and in compliance with best

practices.

8.Manage donation inventory in family suites and collaborate with Volunteer Coordinator to replenish

stock when needed in family suites at Night Shelter. (Diapers, clothing, laundry detergent, etc..)

9.Exceed the minimum expectation of weekly face-to-face meeting with families.

10.Facilitate in-house group meetings and social activities in the community.

11.Attend weekly staffing and community meetings as designated. Uphold professional confidentiality


Requirements:

Requirements:

Previous experience in case management, recovery, or justice work. Or previous experience

working with adults and children living with disabilities.

Ability to use a computer and accurately enter data into a database. HMIS experience preferred.

Pre-employment background check, driving record, and drug test.

Must have a current Oklahoma driver’s license, reliable transportation and updated car

insurance.


Preferred:

Professional experience navigating mental and physical health systems, benefits and

resources

Previous shelter experience Behavioral Health Case Manager II Peer Recovery Support

Specialist or previous work or lived experience in a recovery environment
