Forest Manor Health & Rehab Center is seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our skilled rehabilitation team. We currently have openings for both Full-Time and Per Diem (PRN) positions.
As a Speech-Language Pathologist, you will play a vital role in helping residents improve communication, cognitive-linguistic skills, and swallowing function, enabling them to achieve their highest level of independence and quality of life.

Responsibilities
- Evaluate residents and develop individualized speech-language pathology treatment plans.
- Assess and treat communication disorders, cognitive-linguistic impairments, speech and language deficits, and swallowing disorders (dysphagia).
- Implement evidence-based therapeutic interventions to maximize resident outcomes and functional independence.
- Collaborate with physicians, therapists, nursing staff, dietary services, and families to support resident goals and care plans.
- Educate residents, families, and caregivers regarding communication strategies, swallowing precautions, and home programs.
- Maintain accurate and timely documentation in accordance with facility policies, state regulations, and Medicare guidelines.
- Participate in care planning and interdisciplinary team meetings.
Qualifications
- Current Speech-Language Pathologist license in the State of Alabama.
- Certificate of Clinical Competence (CCC-SLP) preferred.
- Strong clinical, interpersonal, and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Experience in skilled nursing, rehabilitation, or long-term care settings preferred but not required.
