Qualifications

Physical Therapist (PT):

• Current Physical Therapist license in West Virginia or eligibility through applicable licensure requirements

• Graduate of an accredited Physical Therapy program

• Strong communication, organization, and documentation skills

Physical Therapist Assistant (PTA):

• Current Physical Therapist Assistant license in West Virginia or eligibility through applicable licensure requirements

• Graduate of an accredited PTA program

• Strong communication and documentation skills

Certified Occupational Therapy Assistant (COTA):

• Current COTA license in West Virginia or eligibility through applicable licensure requirements

• Graduate of an accredited Occupational Therapy Assistant program

• Strong communication and documentation skills

Responsibilities

• Provide quality therapy services in a skilled nursing and long-term care setting

• Assist residents in achieving individualized rehabilitation goals

• Complete accurate and timely documentation

• Collaborate with therapists, nursing staff, physicians, and other interdisciplinary team members

• Promote resident safety, independence, mobility, and quality of life

• Follow all applicable state and federal regulations and facility policies

• Provide compassionate, resident-centered care
