Job Purpose:

Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. The LPN/RN will report directly to the Director of Nursing.


Skills/Qualifications:

LPN must be 21 years of age with a HS diploma and a Certificate of Practical Nursing and/or Associate Degree, and have a current unrestricted license to work in the state of Missouri and be CPR certified. The LPN/RN must have a minimum of 2 years of direct patient contact with either a Skilled Nursing Facility or an In- Home Care Agency.


Job Responsibilities:

  • Provide skilled nursing interventions in the treatment of the patient’s illness, rehabilitative needs, and preventive care.
  • Utilize a skilled approach in the provision of problem-specific therapeutic interventions, teachings, and training activities in accordance with the plan of care.
  • Assists with admissions and assessments
  • Regularly visits LTC and Hospital facilities to assist with outreach and marketing
  • Develops sales by making initial presentations; explaining products and services; and by performing community outreach. He or she will provide education to the clients, hospital staff, physicians, and community stakeholders on Home Care Services and the admission process.

  • Confer with the RN supervisor regarding needed changes in the Plan of Treatment/Care.
  • Provide hands-on care and patient education utilizing At Home Care Policy.
  • Treat wounds, change wound dressings, give injections, administer enemas
  • Collect specimens from patients
  • Administers medications and sets up medication dispensers.
  • Measures and records vital signs
  • Reports and observes changes in patient conditions to the Director of Nursing
