School-Based Physical Therapist - Chimayo, NM

Key Qualifications:

• Licensed Physical Therapist (PT) with current, valid credentials
• New Mexico Public Education Department (NMPED) license required
• Experience working in school-based or pediatric settings preferred
• Strong communication and collaboration skills to work effectively with students, teachers, and families
• Ability to conduct assessments, develop individualized treatment plans, and document progress according to school and state guidelines

Primary Responsibilities:

• Provide direct physical therapy services to three students, one day per week on a flexible schedule
• Conduct evaluations and collaborate with the educational team to support each student’s Individualized Education Plan (IEP)
• Maintain thorough documentation of student progress and participate in team meetings as needed
• Offer consultative support and recommendations for classroom accommodations
