Registered Nurse (RN) – Home Health Case Manager

Location: Warren, Logan & Simpson Counties, KY
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
On-Call: Every 4th weekend and 1–2 days per week

We are seeking an experienced Registered Nurse (RN) Case Manager who is passionate about delivering exceptional patient care while coordinating services that improve health outcomes.

Compensation & Benefits

Responsibilities

• Complete comprehensive nursing assessments for home health patients.
• Develop, implement, and monitor individualized patient care plans.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Manage transitions of care to ensure continuity and prevent hospital readmissions.
• Educate patients and caregivers on disease management, medications, and treatment plans.
• Coordinate community resources and non-medical support services when appropriate.
• Monitor patient progress and adjust care plans based on changing needs.
• Maintain accurate and timely clinical documentation.
• Promote health, wellness, and preventative care while serving as a patient advocate.
• Ensure compliance with agency policies and regulatory standards.

Requirements

• Active Kentucky Registered Nurse (RN) License
• Associate's or Bachelor's Degree in Nursing
• Current, unrestricted RN license
• Case Management experience preferred
• Strong clinical assessment and critical thinking skills
• Ability to work independently while managing a patient caseload

Preferred Qualifications

• Home Health experience
• OASIS documentation
• Homecare Homebase (HCHB)
• Wound VAC management
• IV therapy
• Lab collection
• PICC line care

Position Details

• Shift: Monday–Friday, 8:00 AM – 5:00 PM
• On-Call: Every 4th weekend and 1–2 weekdays per week
• Coverage Area: Warren, Logan, and Simpson Counties
• Productivity Expectation: 30 points per week
• Mileage Reimbursement: $0.43 per mile
