What You’ll Do
- Monitor and help improve clinical quality measures and patient outcomes
- Identify gaps in care and work with providers and clinical teams to address them
- Collaborate with insurance plans regarding quality initiatives, performance measures, and patient-care needs
- Review patient information and clinical data to identify opportunities for follow-up and intervention
- Support preventive care, screenings, chronic disease management, and other quality initiatives
- Assist providers with clinical needs related to value-based care and quality improvement
- Coordinate patient outreach and follow-up as needed
- Help improve workflows, documentation, and processes that support quality goals
- Serve as a clinical resource to providers and staff
- Work collaboratively across departments to improve the overall patient experience and quality of care
What We’re Looking For
- Current RN license
- Clinical nursing experience, preferably in primary care, family medicine, ambulatory care, care management, or a similar setting
- Experience with quality improvement, population health, care management, HEDIS/quality measures, or value-based care is highly desirable
- Comfortable reviewing clinical data and identifying gaps in patient care
- Strong communication and organizational skills
- Ability to work collaboratively with providers, clinical teams, and insurance plans
- Passion for improving patient outcomes and access to quality healthcare
- Bilingual English/Spanish strongly preferred
