Position: Home Health RN
Schedule: M–F, 8 AM–5 PM + on-call/weekends
On-call: At least 1 weekend/month
License: WV RN
Territory: Harrison, Marion, Taylor, Upshur, Lewis, Doddridge & Barbour counties
Mileage: $0.54/mile
Productivity: 30 points/week
Required: Home health experience, HCHB, OASIS
Main Duties:
Assess patients and implement/evaluate care plans
Provide case management, health maintenance, and disease prevention
Coordinate care with physicians and other healthcare professionals
Manage medications, physician orders, documentation, and patient appointments
Monitor changes in patient condition and communicate with physicians
Coordinate hospital admissions/discharges
Supervise LPN/LVN staff as needed
Train and educate new and unlicensed staff on health and safety procedures
