Field Nurse Supervisor (RN) - Multiple Locations
Job Description
Field Nurse/Home Care nursing experience required
The Regional Field Nurse Supervisor is responsible for delivering high-quality, compassionate nursing care to patients in their homes. Conduct supervisory joint visits with Field Nurses. Provide training/in-service education to professional clinical field staff and paraprofessional staff. Perform nursing duties as assigned in the Diagnostic & Treatment Center (DTC) or Medical Practice. In addition, this role serves as a clinical oversight and quality monitoring resource for home-based clinical services.
Perform routine home visits including:
- Wound care management and dressing changes
- Medication administration and reconciliation
-General health and functional assessments as directed
- Provide nursing care in accordance with the individualized care plan, physician orders, and best nursing practice standards
- Assess patient conditions and monitor changes in health status, promptly communicating updates to the care team and physician as needed
- Educate patients and caregivers on disease management, medication adherence, wound care, and safety in the home environment
- Participate in interdisciplinary case conferences and clinical meetings as
needed
- Perform nursing duties in DTC/Medical practice according to policies and professional nursing scope of practice
- Meet a standardized set of competencies for the specific position
Additional Requirement: Valid NYS driver’s license and reliable vehicle required

Setting: Field and On-site
Schedule: Monday - Friday 8:30am-5:30pm, (1 hour lunch) (FT hours)
Contract Length: Temp to perm opportunity based on performance and business needs
