Job Description
Position: Home Health LPN
Schedule: Monday–Friday
Hours: 40 hours not guaranteed, but work is definitely available
On-call: 1 weekend/month — Saturday or Sunday, not both
License: Illinois LPN
Territory: St. Clair, Madison, Clinton, Randolph, Bond, Monroe & part of Washington County
Mileage: $0.72/mile
Productivity: 32 points/week
Required Skills: Wound care/VACs, lab draws, medication management/education, disease-process education, G-tube, trach, PleurX, drains, nephrostomy, colostomy/urostomy
Main Duties:
Provide care according to individualized care plans
Monitor vitals and report abnormal findings
Administer medications, feedings, oxygen, and other treatments
Perform wound/ostomy/drain care and other skilled nursing tasks
Coordinate with PT, OT, speech, and other care professionals
Maintain infection control and patient safety
Document care, progress, incidents, and outcomes
Order medications, supplies, and feedings as needed
Monitor and maintain medical equipment
