Licensed Practical Nurse

The Licensed Practical Nurse (LPN) plays a vital role in providing comprehensive care within a long-term care facility. Working under the direct supervision of an RN Supervisor and as part of a multidisciplinary care team, the LPN is responsible for delivering high-quality patient care through clinical tasks and attentive collaboration. The role operates on a 3/11 shift schedule and requires proficiency in specific electronic health records (EHR) systems for accurate documentation.

Responsibilities

• Provide direct patient care tailored to individual needs
• Administer medications safely and accurately
• Monitor and record vital signs consistently
• Perform wound care procedures and manage dressing changes
• Maintain detailed and compliant documentation using EHR software
• Collaborate effectively with the multidisciplinary care team

Required Qualifications

• Licensed Practical Nurse (LPN) diploma or certificate from an accredited program
