About the Role

Seeking an experienced and compassionate Director of Social Services to join our team of dedicated professionals. The Director will lead the Social Services department, ensuring each resident receives individualized emotional, social, and psychological support. This role is key in promoting residents’ well-being, guiding transitions, and developing impactful programs that enhance quality of life.

Key Responsibilities

  • Supervise and mentor Social Services staff; delegate and oversee daily department activities

  • Assist residents and families during the admission and transition process to ensure comfort and understanding

  • Complete comprehensive psychosocial assessments for all residents to identify emotional, social, and psychological needs

  • Develop and implement individualized plans of care addressing residents’ social and medical-related needs

  • Maintain accurate resident social history and documentation in accordance with regulations

  • Build and maintain a directory of community resources and support services available to residents and families

  • Plan, organize, and evaluate Social Services programs, including the Guest Survey Program

  • Collaborate with nursing, therapy, and administrative teams to ensure holistic care for residents

  • Lead discharge planning to ensure smooth transitions and appropriate follow-up support

  • Advocate for residents’ rights and best interests in all areas of care and service

  • Conduct interviews and ensure completion of relevant MDS sections (cognitive, mood, behavior, goals) and Care Area Assessments

  • Perform additional duties as assigned to support facility goals and resident care

Qualifications

  • Bachelor’s degree in Social Work or a related human services field (such as sociology, psychology, special education, or rehabilitation counseling) – required

  • Minimum 1 year of supervised social work experience in a healthcare setting working directly with individuals – required

  • Current state licensure meeting Pennsylvania Social Worker requirements – required

  • Experience in skilled nursing or long-term care (SNF/LTC) preferred

  • Strong interpersonal, communication, and organizational skills

  • Ability to work collaboratively in a multidisciplinary team environment
