Job Description
THIS ROLE IS PART TIME WITH THE OPPORTUNITY TO MOVE FULL TIME.
The CNA Hospice role provides compassionate care and support to patients nearing the end of life, focusing on patient comfort and dignity. This position involves close collaboration with an interdisciplinary team, including family members and social workers, to ensure comprehensive care. The CNA Hospice typically manages a caseload of 4-6 patients during standard day shifts, documenting care activities using Electronic Health Records (EHR).
Responsibilities
- Provide direct patient care to hospice patients
- Ensure patient comfort throughout end-of-life care
- Support and communicate with patients' families
- Accurately document care in Electronic Health Records (EHR)
- Deliver compassionate end-of-life support
- Maintain clear communication with interdisciplinary team members
- Collaborate effectively with extended hospice teams, family members, and social workers
Required Qualifications
- CNA Certification
Preferred Qualifications
- Entry-level experience in hospice care
- Certified Nursing Assistant (CNA) License
- Patient care skills
- Basic Life Support (BLS) certification
- Compassionate communication skills
- Time management abilities
- Infection control knowledge
- Strong teamwork skills
