Position Overview
We are seeking a compassionate and motivated Physical Therapist to join an in-house rehabilitation team at a skilled nursing facility in Pahokee, Florida.
This full-time, direct-hire opportunity is ideal for a Physical Therapist who enjoys helping older adults and medically complex residents regain mobility, improve balance, reduce fall risk, and achieve greater independence.

At 40 hours per week, $55 per hour is equivalent to approximately $114,400 annually before overtime or additional incentives. Actual earnings will depend on scheduled and worked hours.
Key Responsibilities
- Evaluate residents’ mobility, strength, balance, coordination, functional abilities, and rehabilitation needs.
- Develop individualized plans of care based on evaluation findings, physician orders, clinical goals, and resident needs.
- Establish measurable short-term and long-term rehabilitation goals.
- Provide skilled physical therapy interventions designed to improve mobility, strength, balance, transfers, gait, safety, and functional independence.
- Conduct timely screenings for residents referred to physical therapy.
- Perform specialized assessments when clinically appropriate.
- Monitor resident progress and modify treatment plans when necessary.
- Provide gait training, transfer training, therapeutic exercise, balance activities, mobility instruction, and fall-prevention interventions.
- Educate residents, families, caregivers, and facility staff regarding treatment plans, safety precautions, mobility techniques, and discharge recommendations.
- Collaborate with physicians, nurses, therapy professionals, social services, and other interdisciplinary team members.
- Participate in care planning, discharge planning, and interdisciplinary meetings as required.
- Complete accurate and timely clinical documentation that supports medical necessity and complies with CMS requirements.
- Follow applicable federal, state, local, facility, and professional standards.
- Maintain accurate billing and treatment records in accordance with organizational procedures.
- Use sound clinical judgment when evaluating, planning, implementing, and following up on resident rehabilitation programs.
- Support quality improvement, resident safety, and positive clinical outcomes.
- Perform other clinically appropriate duties associated with the Physical Therapist position.
Qualifications
Required:
- Degree in Physical Therapy from an accredited physical therapy program
- Active Florida Physical Therapist license or the ability to obtain appropriate authorization to practice in Florida
- Understanding of professional, ethical, and regulatory standards for physical therapy practice
- Strong clinical assessment, treatment-planning, documentation, and communication skills
- Ability to work effectively with older adults and medically complex residents
- Ability to collaborate with an interdisciplinary healthcare team
- Commitment to maintaining continuing education and professional licensure requirements
Preferred but not required:
- Skilled nursing, long-term care, geriatric, inpatient rehabilitation, acute-care, or home-health experience
- Familiarity with NetHealth or a comparable therapy documentation system
- Knowledge of Medicare, CMS documentation, and medical-necessity requirements
- Experience with gait training, transfer training, balance interventions, fall prevention, and post-operative rehabilitation
New graduates with relevant clinical rotations, strong communication skills, and an interest in geriatric or post-acute rehabilitation are encouraged to apply.
