Concord Village Rehabilitation and Wellness is seeking a Full-Time Speech-Language Pathologist (SLP) to join our in-house rehabilitation team in Concord, Ohio. PRN opportunities are also available!

What You’ll Do- Evaluate and treat residents with speech, language, cognitive-communication, and swallowing disorders
- Develop individualized plans of care based on each resident’s needs and goals
- Provide skilled SLP services to short-term rehabilitation and long-term care residents
- Collaborate with PT, OT, nursing, physicians, dietary, and other members of the interdisciplinary team
- Provide resident, family, and caregiver education
- Complete accurate and timely clinical documentation
- Participate in discharge planning and interdisciplinary care conferences as appropriate
- Current Ohio SLP license in good standing
- Graduate of an accredited Speech-Language Pathology program
- SNF/LTC experience preferred
- Experience with Net Health and PointClickCare (PCC) preferred
- Strong clinical, communication, and documentation skills
- Team-oriented and resident-focused approach
- Full-time position with PRN opportunities also available
