Green Meadows Health & Wellness Center is seeking a PRN Occupational Therapist (OT) to join our in-house rehabilitation team in Louisville, Ohio.

What You'll Do

As a PRN Occupational Therapist, you will evaluate and treat residents with the goal of maximizing independence, safety, and overall quality of life.

Responsibilities include:

• Perform occupational therapy evaluations and develop individualized plans of care
• Provide skilled OT interventions focused on ADLs, functional mobility, strength, safety, and independence
• Establish appropriate goals and monitor resident progress
• Complete accurate and timely documentation using Net Health
• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team
• Supervise and provide clinical guidance to COTAs as appropriate
• Educate residents, families, and caregivers regarding treatment recommendations
• Assist with discharge planning and safe transitions
• Maintain compliance with facility, state, federal, and professional standards

Qualifications

• Graduate of an accredited Occupational Therapy program
• Current Ohio Occupational Therapist license in good standing
• SNF/LTC experience preferred
• Familiarity with Net Health and PointClickCare (PCC) is a plus
• Strong communication, documentation, and clinical skills
• Dependable, flexible, and committed to providing quality resident-centered care
