Licensed Practical Nurse (LPN)

Nyack Ridge Rehabilitation & Nursing Center is seeking compassionate, dependable, and skilled Licensed Practical Nurses (LPNs) to join our nursing team.

Available Shifts:

Full-Time

• Day Shift: 7:00 AM – 3:00 PM

• Evening Shift: 3:00 PM – 11:00 PM

Part-Time

• Night Shift: 11:00 PM – 7:00 AM

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans.

• Administer medications and treatments as ordered by physicians.

• Obtain and document vital signs and other clinical information.

• Perform treatments, wound care, and other nursing procedures within the LPN scope of practice.

• Maintain accurate and timely nursing documentation.

• Communicate effectively with physicians, RNs, CNAs, residents, families, and other members of the interdisciplinary team.

• Supervise and provide guidance to Certified Nursing Assistants (CNAs) during assigned shifts.

• Participate in shift reports and communicate important resident information to incoming nursing staff.

• Follow infection prevention, medication administration, safety, and resident confidentiality requirements.

• Maintain compliance with facility policies and applicable federal and New York State healthcare regulations.

• Promote a safe, respectful, and compassionate environment for residents.

Qualifications

• Current and valid New York State LPN license required.

• Graduate of an approved Practical Nursing program.

• Long-term care, skilled nursing, rehabilitation, or related nursing experience preferred.

• Strong clinical assessment and medication administration skills.

• Excellent communication and documentation skills.

• Ability to prioritize responsibilities and respond effectively to changing resident needs.

• Strong organizational and time-management skills.

• Compassionate, dependable, and resident-focused.

• Ability to work collaboratively with CNAs, nurses, physicians, and other healthcare professionals.
