Job Description
Nyack Ridge Rehabilitation & Nursing Center is seeking compassionate, dependable, and skilled Licensed Practical Nurses (LPNs) to join our nursing team.
Available Shifts:
Full-Time
Day Shift: 7:00 AM – 3:00 PM
Evening Shift: 3:00 PM – 11:00 PM
Part-Time
Night Shift: 11:00 PM – 7:00 AM

Responsibilities
Provide direct nursing care to residents in accordance with individualized care plans.
Administer medications and treatments as ordered by physicians.
Obtain and document vital signs and other clinical information.
Perform treatments, wound care, and other nursing procedures within the LPN scope of practice.
Maintain accurate and timely nursing documentation.
Communicate effectively with physicians, RNs, CNAs, residents, families, and other members of the interdisciplinary team.
Supervise and provide guidance to Certified Nursing Assistants (CNAs) during assigned shifts.
Participate in shift reports and communicate important resident information to incoming nursing staff.
Follow infection prevention, medication administration, safety, and resident confidentiality requirements.
Maintain compliance with facility policies and applicable federal and New York State healthcare regulations.
Promote a safe, respectful, and compassionate environment for residents.
Qualifications
Current and valid New York State LPN license required.
Graduate of an approved Practical Nursing program.
Long-term care, skilled nursing, rehabilitation, or related nursing experience preferred.
Strong clinical assessment and medication administration skills.
Excellent communication and documentation skills.
Ability to prioritize responsibilities and respond effectively to changing resident needs.
Strong organizational and time-management skills.
Compassionate, dependable, and resident-focused.
Ability to work collaboratively with CNAs, nurses, physicians, and other healthcare professionals.
