PRN Physical Therapist
Assess, plan, organize, and provide rehabilitative services that improve mobility, relieve pain, increase strength, and prevent or correct disabling conditions resulting from illness or injury.
As a PRN Physical Therapist, you will:
- Perform and document initial evaluations, analyze clinical findings, identify patient needs, and establish a diagnosis before intervention.
- Develop and implement individualized treatment plans to maintain, improve, or restore physical function, alleviate pain, and prevent further dysfunction.
- Evaluate patients’ responses to treatment and adjust plans of care to achieve maximum benefit.
- Apply infection-control standards and universal precautions while providing patient care.
- Ensure continuity and quality of care through accurate, timely documentation.
- Communicate effectively with patients, families, team members, physicians, and other healthcare professionals.
- Promote and uphold the agency’s philosophy, policies, and procedures.
- Complete all required documentation within company-established guidelines.
- Accept patient assignments based on availability and organizational needs.
- Successful completion of an accredited physical therapy program.
- Current, valid Physical Therapist license in the state of practice.
- Minimum of one year of home health experience preferred.
- Current CPR certification.
- Valid driver’s license and automobile insurance.
- Homecare Homebase experience preferred.
- Ability to work a flexible PRN schedule based on patient-care needs.
- Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods.
- Ability to lift, carry, pull, or push up to 100 pounds.
- Ability to travel to patients’ homes within the assigned service area.
