About the Role:
As a Licensed Practical Nurse, you will be responsible for providing high-quality, patient-centered care to our residents. Your primary responsibilities will include:
Assessing residents' physical, emotional, and psychological needs
Developing and implementing individualized care plans
Administering medications and treatments as prescribed by physicians
Monitoring residents' conditions and reporting changes to healthcare providers
Collaborating with interdisciplinary teams to ensure comprehensive care
Responsibilities:
Provide direct care to residents, including personal care, wound care, and assistance with activities of daily living
Monitor and maintain accurate records of residents' care
Participate in quality improvement initiatives to improve patient outcomes
Develop and maintain relationships with residents, families, and interdisciplinary teams
