Evening Shift LPN

Job Summary:

Join Willow Springs Memory Care, a person-centered memory care company dedicated to providing high-quality care, comfort, and support to individuals living with cognitive impairment and their families. As an Evening Shift LPN, you will play a vital role in creating a warm and inclusive environment for our residents. We are seeking a compassionate, skilled, and dedicated LPN to join our dynamic team.

Responsibilities:

  • Provide direct care to residents, including administering medications, performing vital signs, and assisting with daily living activities

  • Develop and maintain personalized care plans, working closely with the care team to ensure resident needs are met

  • Assist with mealtime, including serving, feeding, and ensuring hydration needs are met

  • Participate in care conferences and contribute to resident assessments, care plans, and service plans

  • Maintain accurate and timely records of resident care and services

  • Collaborate with other healthcare professionals, including physicians, CNAs, and staff members to ensure high-quality care

  • Engage with residents, families, and staff members to promote a positive and inclusive environment

  • Perform other duties as assigned by the shift leader or director of nursing

Preferred Qualifications:

  • Current LPN/LVN license in the state of California

  • One year of experience in a long-term care setting

  • Basic Life Support (BLS) certification

  • Ability to lift, push, and pull up to 50 pounds

  • Strong communication and interpersonal skills
