Concord Village Rehabilitation and Wellness is seeking a PRN Physical Therapist (PT) to join our rehabilitation team in Concord, Ohio. What You’ll Do

• Evaluate residents and develop individualized physical therapy plans of care
• Provide skilled PT services to short-term rehabilitation and long-term care residents
• Help residents improve strength, balance, mobility, endurance, and overall function
• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team
• Complete accurate and timely documentation
• Participate in discharge planning and resident/family education
• Maintain compliance with facility, state, federal, and professional standards

What We’re Looking For

• Current Ohio Physical Therapist license in good standing
• Graduate of an accredited Physical Therapy program
• SNF/LTC experience preferred
• Experience with Net Health and PointClickCare (PCC) preferred
• Strong communication and documentation skills
• Dependable, collaborative, and resident-focused approach
• Flexibility to assist with PRN coverage based on facility needs
