Job Summary:

Responsibilities:
Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming
Assist with medication management and administration under the supervision of a licensed nurse
Prepare meals and snacks, and assist with light housekeeping and laundry
Monitor and report changes in client condition to the nursing team
Maintain accurate records of client care and services provided
Develop and maintain strong relationships with clients and their families
Collaborate with other healthcare professionals to ensure comprehensive care
Requirements:
High school diploma or equivalent required
Completion of a Home Health Aide (HHA) training program or PCA certification
1+ year of experience in a home care or related setting
Ability to lift, transfer, and reposition clients, with or without the use of assistive devices
Strong communication and interpersonal skills
Ability to work independently and as part of a team
