Job Description
Location: Connecticut Shoreline
Compensation: $55–$65 per hour
Job Type: Full-Time / Part-Time / Per Diem
Service Area: Old Lyme to Madison, Connecticut
We are seeking a motivated and compassionate Physical Therapist (PT) to join our team and provide high-quality therapy services throughout the Connecticut Shoreline, covering the area from Old Lyme through Madison.
This is an excellent opportunity for a PT who enjoys building meaningful relationships with patients while providing individualized care in a community-based setting.
What You'll DoConduct comprehensive physical therapy evaluations.
Develop individualized treatment plans based on patient needs and functional goals.
Provide skilled physical therapy interventions to improve strength, mobility, balance, coordination, and functional independence.
Monitor patient progress and modify treatment plans as clinically appropriate.
Educate patients and caregivers on exercises, safety, mobility, and home programs.
Coordinate care with physicians, nurses, care managers, and other healthcare professionals.
Maintain timely and accurate clinical documentation.
Communicate patient progress and concerns to the appropriate members of the care team.
Provide services throughout the assigned Shoreline territory.
Maintain compliance with Connecticut licensing and applicable healthcare regulations.
Current and valid Connecticut Physical Therapist license.
Graduate of an accredited Physical Therapy program.
Current CPR/BLS certification, as required.
Strong clinical, communication, and documentation skills.
Ability to work independently and manage a flexible schedule.
Reliable transportation for community-based assignments.
Previous home health, outpatient, geriatric, or community-based experience is a plus.
