Responsibilities:
Provide direct patient care to our residents, including administering medications, performing vital signs, and assisting with daily living activities
Develop and implement individualized care plans in collaboration with our interdisciplinary care team
Conduct regular assessments to monitor patient progress and adjust care plans as needed
Communicate effectively with patients, families, and colleagues to ensure seamless transitions and optimal care outcomes

Requirements:
Graduate of an accredited Licensed Practical Nurse (LPN) program
Current LPN licensure in the state of Pennsylvania
Ability to work varied shifts, including days, evenings, and weekends
Strong communication and interpersonal skills
