Responsibilities:
Provide high-quality, patient-centered care to residents in our post-acute rehabilitation and long-term care settings
Administer medications, treatments, and procedures under the supervision of a registered nurse
Monitor and report changes in resident condition to the care team
Collaborate with healthcare professionals to develop and implement individualized care plans
Maintain accurate and detailed records of resident care and treatments
Provide emotional support and assistance with activities of daily living (ADLs) as needed
