LPN Care Coordinator - Memory Care Unit

Job Summary:

We are seeking a compassionate and skilled LPN Care Coordinator to join our team at Eastern Star Home in Oriskany, NY. As a key clinical leader, you will be responsible for the overall coordination, clinical oversight, and day-to-day management of our 14-bed Memory Care Enhanced Assisted Living Unit. This is a LPN Care Coordinator - Memory Care Unit opportunity to make a meaningful difference in the lives of our residents.

Responsibilities:

• Coordinate and manage the care of 14 residents with dementia and other cognitive impairments, ensuring individualized care plans are developed and implemented

• Provide clinical oversight and supervision to nursing staff, including assessment, planning, and evaluation of resident care

• Collaborate with residents, families, physicians, and other healthcare professionals to ensure continuity of care and resolve any issues that may arise

• Develop and maintain relationships with residents, families, and interdisciplinary team members to promote a positive and supportive environment

• Identify and report any potential concerns or safety issues to leadership and regulatory agencies as necessary

• Participate in quality improvement initiatives to ensure high-quality care and service to our residents

Requirements:

• Current NY State LPN license

• Minimum 0 years of experience in assisted living or skilled nursing facility setting

• Strong understanding of dementia care and memory impairment

• Excellent communication and interpersonal skills

• Ability to work effectively in a fast-paced, dynamic environment
